To apply for testing, you must complete the Test Application. If the student is under 14, they will also need to submit a Home Report Card (middle row) that has been graded by a parent. In addition, school age students, must submit a copy of their latest School Report Card.
If you already turned in your report card, do not turn it in again. If you are a beginner and do not have your last school report card,
turn it in at your next test. APPLICATIONS MUST BE PRINTED IN COLOR.
